All students and their parents need to meet with administration and complete the following documents:

• Application Form

• Statement of Faith (Parent/Student)

• Honor Code Pledge (Parent/Student)

• Dress Code Agreement (Parent/Student)

• Birth Certificate

• Immunization Records

• Emergency Card

• Last 2 report cards, if available

• Achievement Test Scores, if available

• Financial Agreement
